i was just running your basic Promo Action in "safe"-mode and just did 50 follows and 50 likes per day with about a minute of pause inbetween. (i think thats a very conservative number?)
But it always asks me to verify that i am human with this video authentication, after only maybe 20 - 30 follows /likes.

I only use 1 Account.

What am i doing wrong?

The video challenge is a new(ish) thing. As to why it affects some accounts and not others, at this time I don't have an answer.

Damn...thats not good. Do you think it might be worth it to wait till instagram changes something? Or is this account "lost"?
That would be really bad because a lot of real work went into this one.

I wont do anything for a couple of days so i dont get banned now.

Update: I tried it again with a brand new account. Created by hand. New handmade email used.

After the first 20 Follows i have to verify...

What am i doing wrong?

because its new. just profile and watch manually few stories 5-10
